The department of anesthesiology at the University of Florida College of Medicine – Jacksonville offers a one-year regional anesthesiology and acute pain management fellowship program, which focuses on comprehensive treatment of perioperative pain in patients at UF Health Jacksonville. Fellows also treat patients at the attached outpatient surgery center and at the all-new UF Health North facility.

Fellows rotate between these three locations, working individually and with residents to coordinate care and complete blocks. Fellows master both single-shot and catheter-based techniques used for upper- and lower-extremity procedures. As a Level 1 trauma center, many of our techniques are employed for fracture surgery. By the end of their training, fellows will be familiar with common orthopaedic procedures used to treat this unique patient population. A host of other techniques are employed during the one-year fellowship.

University of Illinois at Chicago Program

University of Illinois at Chicago offers a 12-month ACGME-accredited fellowship in Regional Anesthesia. The fellowship offers extensive experience in performing regional anesthesia techniques and managing patients who undergo regional procedures. Multiple faculty members with diverse experiences offer a high level of clinical training. Our department consists of supportive and engaged colleagues who seek to maximize the fellows’ professional growth during their year of training with us.

Training
Fellows will rotate through Regional Anesthesia, Acute Pain Service, and Chronic Pain rotations during their fellowship. They will perform a high volume of blocks, which include a wide variety single-shot blocks (such as supraclavicular, interscalene, femoral, PENG, popliteal), neuraxial procedures (including combined spinal-epidurals, thoracic epidurals, and continuous nerve catheters. We seek to incorporate newer truncal techniques into our service, such as erector spinae, TAP, QL blocks. Fellows also manage patients with epidural and peripheral nerve catheters as part of the Acute Pain Service.

Furthermore, with the opening of the new Specialty Care Building (SCB), fellows will now have experience in a high throughput regional anesthesia outpatient surgical center. As the year progresses, fellows gain increasing independence and the focus shifting more towards supervision of residents and trainees and organizing / running the Acute Pain Service. As an ACGME-accredited fellowship, we follow guidelines for training as stated on the ACGME website.

There is a strong scholarly focus at our program with fellowship participation in journal clubs, case conference, and grand rounds. Active participation and presentation in national meetings, such as those put on by the American Society of Regional Anesthesia (ASRA), is facilitated by faculty members.

Fellows may choose to spend time moonlighting as an attending in the Main OR supervising residents and CRNAs for general anesthesiology cases. This experience, for fellows who choose it, aids in growth as a physician in a supportive environment.

Goals
At the completion of our program, graduating fellows will be able to:

• Master a wide range of regional procedures, including continuous nerve blocks and epidural catheters
• Manage an Acute Pain Service, which includes rounding and optimizing management of patients with epidural and peripheral nerve catheters
• Demonstrate excellent skills in patient safety and communication to manage a Regional Anesthesia service to perform procedures safely and efficiently
• Practice evidence-based medicine to integrate new concepts and technologies into practice of Regional Anesthesia
• Practice in a multi-disciplinary setting and work with other anesthesiologists, surgeons, nurses, pharmacists, physical therapists, other medical professionals, operating room managers, and hospital administrators.

Starting with the 2024 – 2025 Fellowship year, we will be participating in the national RAAPM match through SFMATCH (www.sfmatch.org).

Description

The goal of the fellowship program is for the fellow to be exposed to the advanced interdisciplinary evaluation and management of acute, chronic, and cancer-related pain, and to procedures related to the treatment of these patients. Upon completion of the fellowship, the fellow will be able to function as an independent, board eligible pain management physician and consultant, competent in the interdisciplinary evaluation and treatment (including advanced interventional techniques) of complicated and routine pain problems. We provide multidisciplinary assessment and treatment of the full spectrum of ambulatory pain patients. To do this effectively, we have a facility and staff dedicated to pain management. We know that ongoing pain changes a person’s life in all areas, and we are here to help. Our approach is individualized treatment because we know every person experiences pain in a different way. Our team of pain specialists look at the whole person to evaluate the pain problem and develop a plan with the patient and their referring physician to improve their quality of life and pain control. We are unique because we bring together a dedicated group of professionals to provide state-of-the-art pain assessment and treatment, research into new treatments, and coordination with patients’ health care providers. As the only academic pain center in Iowa, we bring expertise to all pain patients. We provide consultation to patients and physicians not just in Iowa City, but throughout Iowa and beyond. Description

The University of Kansas Multidisciplinary Pain Medicine Fellowship is an ACGME-accredited program administered within the department of anesthesiology. The vision of this program is to provide the most advanced training in interventional pain medicine in the country. Fellows will receive multi-disciplinary training with rotations through physical medicine & rehabilitation, neurology, and psychiatry during the course of the 12-month program. The program accepts two fellows a year from an American Board of Medical Specialties accredited residency program. The core faculty of the fellowship includes six board certified pain physicians whose practice primarily involves interventional treatments to patients suffering from a variety of painful conditions. Our pain center has also been named a Center of Excellence for radiofrequency ablation for spinal cancer as well as one of the leading centers in the nation for utilization of spinal cord stimulation and intrathecal pumps for select patients with intractable pain. Common treatments provided include epidural steroid injections, facet injections, spinal cord stimulation, intrathecal pump implantation, vertebral augmentation, kyphoplasty, radiofrequency ablation, neurolytic blocks, and a multitude of other interventions. Fellows will become adept at the use of fluoroscopy and ultrasound to provide accurate and effective treatments. Significant exposure to cancer pain management and advanced interventional techniques for the management of its pain is one of the cornerstones of the fellowship. Fellows will be encouraged and provided ample opportunity to participate in research and attend at least one national pain meeting during the fellowship year.

The University of Maryland Regional Anesthesiology Acute Pain Medicine Fellowship is a full-time fellowship position.  Clinical experience is divided between the University of Maryland Medical Center's main campus, the Shock Trauma Center and the University of Maryland Rehabilitation and Orthopedic Institute (UMROI).  UMROI has busy total joint and outpatient sports medicine practices.  Peripheral nerve catheters are regularly employed, giving fellows the opportunity to learn about their management in both the inpatient and ambulatory settings.  At the main campus and Shock Trauma, fellows provide all types of regional anesthetics including lumbar plexus blocks, paravertebral blocks, serratus/pec blocks and cranial nerve blocks. Fellows perform or supervise residents performing about 1500 blocks per year.

There is also ample opportunity for scholarly endeavors. Fellows receive one day per week of nonclinical time and two months of elective time. Fellows are expected to engage in a research or quality improvement project with a faculty mentor, with the goal of publication or presentation at a national meeting.  Fellows also participate in our twice annual cadaver dissection program.  Fellows participate in regular didactic lectures, journal clubs and our nationally attended annual regional anesthesia conference.  We have an experienced faculty, including nationally and internationally recognized experts in the field of Regional Anesthesia for adults and children.

The University Of Nebraska Medical Center Department Of Anesthesiology Fellowship for Acute Pain Medicine and Regional Anesthesia is a one year ACGME accredited program. Our Regional Anesthesiology and Acute Pain Medicine division offers 2 ACGME accredited fellowship positions. You will become an expert at regional anesthesiology and an experienced consultant as part of the Acute Pain Service.

The bulk of your rotations will take place at our ambulatory surgery center where we perform thousands of nerve blocks and outpatient catheter placements. In addition, you will rotate on the Acute Pain Service at our main hospital. As the only Level 1 Trauma Center and referral hospital for a large geographic area, you will be exposed to complex patients requiring skillful incorporation of regional anesthetic and acute pain management techniques in addition to exposure to pediatric nerve blocks and catheter management via our associated Children's Hospital.

The Ottawa Hospital is the major tertiary centre for the Nation’s capital and, the primary teaching centre for the University of Ottawa. It is home to the largest non military simulation centre in North America. The Regional Anesthesia Fellowship program is well established and nationally recognized. Our subspecialty core group has received Regional Anesthesia Fellowship training from a variety of locations: Brigham and Women's Harvard University, Duke University, The Monash University in Australia, The Virginia Mason Center in Seattle, The University of Toronto, University of Alberta and locally at the University of Ottawa. Our members have been awarded teaching awards for outstanding contributions to Resident, Fellow and Medical student teaching and administration in Regional Anesthetic Techniques. Former graduates of our Fellowship training program have been instrumental in the development of their own Regional Anesthesia training in their home base nationally and internationally. Our Fellowship program is one year in duration. Two Fellowship positions are open for each academic year that spans from July 1 to June 30. You will be based at either the Civic or General Campus.

The University of Pennsylvania’s regional anesthesiology and acute pain medicine fellowship is a year-long post-graduate training program aimed to cultivate the expertise in the art, theory, and practical delivery of regional anesthesia and acute pain medicine. Our fellowship is currently non-ACGME accredited, but there are plans of making it ACGME accredited.

Our institution has two primary hospitals (Penn Presbyterian Medical Center – our level 1 trauma center – and the Hospital of the University of Pennsylvania) and a free standing ambulatory surgical center (Penn Medicine University City), situated in the urban setting of Philadelphia, in the University City neighborhood. We perform approximately 450 blocks/month (~5400 blocks/year) with a healthy mix of single-shot peripheral nerve blocks, truncal blocks, continuous catheter nerve blocks, and neuraxial analgesic blocks/catheters. We routinely provide regional anesthesia and acute pain medicine services to a variety of surgical specialties including orthopedics, trauma, podiatry, plastics, breast, cardiac, vascular, colorectal, general surgery, gynecology, transplant, and ambulatory surgeries. We also have a dedicated acute pain service that manages continuous nerve block catheters, epidural catheters, acute pain medication management, and complex acute on chronic pain medication management.

We have a core group of dedicated regional anesthesia and acute pain medicine attending anesthesiologists who will be providing education throughout the year. Your education includes hands-on training in ultrasound guided peripheral nerve blocks, management of peripheral nerve continuous catheters, neuraxial anesthesia/analgesia, and multimodal pharmacological therapies for postoperative pain management. There are dedicated monthly lectures geared towards advanced regional anesthesia and acute pain medicine topics for our fellow, along with routine, weekly lectures given for anesthesia residents who rotate on their regional anesthesia and acute pain medicine rotation. These lectures are round table discussions or hands on training sessions with the core regional anesthesia and acute pain medicine attendings to help solidify knowledge and gain expertise in this field. Journal clubs are held throughout the year to discuss the latest evidence in the field of regional anesthesia and acute pain medicine. Finally, you will be required to pick a topic of interest in the earlier portion of your fellowship year, to perform a quality improvement project. This will be a great opportunity to gain proficiency in using systematic, data-guided initiatives to improve clinical care, patient safety, or health care operations related to the field of regional anesthesia and acute pain medicine.

As this fellowship is currently non-ACGME accredited, this fellowship currently incorporates a part-time clinical instructorship with the responsibility of supervising residents and CRNA’s in the operating room setting.

At the end of your year, you will be very well trained in the field of regional anesthesiology and acute pain medicine, and we are confident that you will succeed in your career whether it is in the academic or private sector.

  This Regional Anesthesiolo-gy Fellowship Program consists of 12 months of subspecialty training for qualified physicians who have completed an anesthesiology residency. Preference is given to mature individuals with significant experience in regional anesthesia who are interested in becoming an “expert” in acute pain and regional anesthesia. Fellowship training is not only concerned with the development of expertise in the practice and theory of regional anesthesiology and acute pain management techniques, but also the understanding of the related physiology and pharmacology, patient outcomes, and financial related issues. Cases: Fellows are involved in the acute postoperative interventional pain service, performing, under supervision, lower and upper extremity, paravertebral blocks, thoracic wall blocks (PECS I, PECS II, Serratus Anterior), Quadratus Lumborum and TAP blocks.  Blocks involve both single shot and continuous techniques. Emphasis is placed on ultrasound guided and neuro-stimulation techniques.  Extensive experience in pediatric and obstetrics is available through the UPMC Children’s Hospital of Pittsburgh and UPMC Magee- Womens Hospital.  Management of patients into the postoperative period in association with ERAS protocols is emphasized.

The fellowship is ideal for trainees who want to become efficient in performing peripheral and truncal blocks. Each Regional fellow is expected to perform over 1,000 blocks annually. Fellows are required to attend weekly formal lectures; monthly journal clubs and pediatric regional ultrasound workshops. The fellowship has incorporated all ACGME program requirements into the training program.  Potential fellows should expect to far exceed all minimum procedural and experience requirements in the areas of regional, acute, trauma, chronic and pediatric pain. The total number of blocks available at all sites to the Regional and Acute Pain fellows is over 42,000 which includes 4103 continuous nerve blocks and 8336 spinals. The regional anesthesia experience is divided amongst the sites to maximize the variety of experiences. For instance, UPMC Mercy and UPMC Presbyterian provides extensive experience in trauma and burns. UPMC Passavant provides extensive experience in paravertebral nerve blocks and lower extremity blocks. UPMC Children’s Hospital of Pittsburgh provides a pediatric site and the ambulatory sites provide extensive experience in extremity nerve blocks. At each site the fellows train under the direct supervision of a Regional Anesthesia and Acute Pain faculty. The faculty is responsible for clinical and didactic education, providing feedback and evaluation of fellow knowledge and level of technical skills.

Fellows will also attend an annual fresh cadaver dissection workshop, present at local, regional and national meetings, and participate in clinical research.

Clinical Training:
The University of Rochester Regional and Acute Pain fellowship provides training experience in both the inpatient and ambulatory surgery center settings.  Strong Memorial is a large University Hospital and Level I Trauma Center, which provides the opportunity to perform a wide range of advanced peripheral nerve blocks and catheters as well as thoracic epidurals for complex surgical and trauma patients.   Strong Memorial Hospital also has a growing inpatient Acute Pain Service, which manages perioperative nerve blocks and acute pain consults. We have multiple collaborations with the departments of orthopedics, OB-GYN, and cardiothoracic surgery in order to deliver effective opioid sparing postoperative pain management.  In addition, fellows spend time at Sawgrass Surgical Center, which is a busy, efficient ambulatory surgical center. At Sawgrass we perform a high volume of peripheral nerve blocks for orthopedic surgery patients, as well as podiatry and breast reconstruction patients.

 Educational Activities:
The fellow will participate in a variety of educational didactics including a regional and acute pain lecture series, oral board preparation, departmental Grand Rounds, and a regional and acute pain journal club. There are also opportunities to teach workshops at departmental educational seminars or at a multi-institutional local/regional conference, present posters at national meetings, attend a full day POCUS seminar, etc.

Academic Educational and Research Projects:
Fellows will be expected to participate in academic projects relating to education and/or research. There are opportunities to participate in ongoing research projects such as outcomes in regional anesthesia, acute pain management for patients with a history of addiction, and several quality improvement initiatives.  Fellows have designated elective time that may be used to pursue clinical or academic interests.

Financial Opportunities and Fellowship Goals:
The fellow has no call or attending responsibilities unless they choose to pursue the 20% attending track with the appropriate increase in compensation. Attending moonlighting shifts are also available if desired. The goal of the fellowship is for trainees to become experts in performing safe, efficient nerve blocks and to be competent in advanced block techniques and acute pain management. Upon fellowship completion, the fellow will be fully prepared for future work in either private practice or academics.
